Employer Matching Gift Program

Will your employer join you in supporting the work of UMMS? Your gift to UMMS Foundation could be doubled or even tripled if your employer has a matching gift program. Here's how it works:

  1. In the box below, enter the company name where you or your spouse are employed. If the company name appears in the list, then they match contributions. If their name is not listed, please check with your human resources or personnel office to see if your employer matches gifts to hospitals or other health care organizations.
  2. Get the appropriate Matching Gift Form from your human resources or personnel office.
  3. Fill out the form completely.
  4. Mail it with your contribution (or a reference to your recent contribution) to UMMS Foundation, 110 South Paca Street, 9th Floor, Baltimore, MD 21201
